So we kept track the only way we could. Folders of prescriptions. Photos of reports on our phones. A call at eight in the morning to ask if she'd taken her medicine, and another at night. And every time a doctor changed, the first question was the same — what's her history? — and every time we searched through twenty years of paper, and still missed something.
No one was careless. Her doctors cared. We cared. The care just ended where the visit did.
